Assign stable ids to stored tokens, migrate legacy ISO lastNotifiedAt
to epoch ms, replace setLastNotifiedAt with db.update, and persist
lastNotifiedAt only after a successful FCM send. Extend Device with
optional lastNotifiedAt (ms).
