fix spacing in a doc diagram

Describe VITE_DEFAULT_NOTIFY_API_SERVER / DEFAULT_NOTIFY_API_SERVER,
the debug-override resolution order, and replace outdated APP_SERVER
assumptions in build and notification testing docs.
