The first four questions allow you to respond free form, the next 20 questions are multiple choice, and the last question is free form. Please provide additional comments where appropriate.

Your answers will be combined with the answers of others who were rating this person, and reflected in a report, but nothing in the report will identify you. So the person you're rating will NOT see your specific answers. The report will also show the comments, but the report will NOT attribute comments to any individual.
